Yes, if you don’t stop them — and it’s the costliest mistake when starting out. Your theme keeps its own buy buttons (the one on the product page, the bar that appears as you scroll, and quick-add on collection pages), and those add the frame with no lenses and no prescription.

Yes, on any Online Store 2.0 theme — the ones that let you add blocks from the theme editor. Optical Form uses a theme-agnostic layer that detects the selected variant and bundles the cart correctly: verified on Dawn and Horizon, with adapters for custom themes. See Theme integration.

No. It installs by enabling an app embed and assigning the form to a product. Only heavily customized themes may need a CSS selector tweak. See Install.

As real Shopify products linked to options (add-ons). They’re added as separate lines and grouped under the main product as a bundle, so Shopify includes them on the order and they reach your lab.

Saving doesn’t publish. You must click Sync for changes to reach the storefront, and the form must be Active. See Sync.

Yes. Export the form as JSON from the Settings tab and use Import from JSON on the other store.

One detail that decides when to subscribe: the trial starts when you approve the subscription, not when you install the app. The days you spend deciding cost you nothing.

Yes, and they’re included in Pro — there’s no future plan to wait for. On install you already have a sample Contact lenses form, in draft, and Create form ships that template.

The customer picks the pack, marks which eyes they’re ordering and their per-eye power. If your product carries the catalogue of combinations the manufacturer actually makes, the app resolves the exact reference and sends it on the order, so your lab can order without translating anything.

It never reaches our servers. The prescription and any file your customer uploads travel over TLS straight into your store’s Shopify order, as line item properties. We don’t store them and couldn’t read them: the app doesn’t request permission to read orders or customers.

That means you are the controller for that data, and access or deletion requests are settled inside your Shopify. You can hide it from what the customer sees without losing it from the order — see Design & cart — and the legal detail is in the Privacy Policy.

Only if you need it, and always to your store’s Shopify address — never to your customers.

Two things: a few setup nudges if you get stuck on a step (three at most, and none at all if you finish setting up), and billing notices if your subscription stops being active. There’s no newsletter, no offers, no weekly cadence.
