Install Optical Form

Optical Form installs through a theme app embed — no code required. The app includes a Setup guide that walks you through these steps.

Install Optical Form from the Shopify App Store. On first open, the setup guide appears on the Home.

  1. In your Shopify admin: Online Store → Themes → Customize.
  2. Open App embeds in the left panel.
  3. Turn on Optical Form Drawer and click Save.

This is the step that makes the button appear, and it’s different from the previous one:

Enable only the embed and the form exists, but the product page shows nothing.

  1. In the theme editor, switch the view at the top to Product (the product template).
  2. In the main section, click Add block → pick Optical Form.
  3. Place it where you want it (usually next to the add-to-cart button) and click Save.

You don’t start from nothing. On install, the app leaves four draft forms built and in your store’s language:

  • Prescription glasses, full screen — with sample treatment colours.
  • Prescription glasses in a side panel — with branches for single vision, progressive, reading and non-prescription.
  • Sunglasses in dark mode.
  • Contact lenses — lens pack, prescription per eye, and care solutions.

They’re drafts, so they don’t appear on any product page until you activate them. Open them, walk through them, and keep whichever matches what you sell — it’s almost always faster than starting blank.

Lens types, treatments and services are billed as real Shopify products. Create them with the Unlisted status — purchasable but absent from collections and search, see Before you start— and link them to options as add-ons.

On the form’s Products tab, choose which products it appears on. On those pages the customer sees the “Configure your lenses” button and follows the multi-step flow.

You can pick individual products or whole collections: with a collection, every product in it — including those that join later — shows the form, no matter how many there are. The rules are in Products and collections.

Set the form to Active and click Sync to publish it to Shopify. A form only appears on the store if it’s Active + Synced. See Sync.

One decision left, and it is a business one: your theme keeps its own buy buttons, so as things stand a customer can walk off with the frame and no lenses. If you also sell bare frames, that is exactly what you want. If you don’t, that order arrives paid and with no prescription, and you find out in the lab.
